Besides mini hamburgers and gourmet mac and cheese, any suggestions for appetizers or even cocktails that follow the theme? (Or, heck, any other ideas we could use to take the theme a notch further?)

To extend the theme have grab/favor bags, sippy cups, streamers & balloons, and perhaps a pinata.
For food, try mini-pizzas, adult grilled cheese, and, of course, cake.

You could use lolly pops as drinks stirrers. You could also make everyone wear candy jewelry and serve your food in a happy meal or in lunch boxes.

My kids love orange juice or pretty much any purple drink. A Mimosa (orange juice with champagne) would be a great play on the OJ and a Plum Fizz ( 2 oz. soda water, .5 oz lime juice, .5 oz super-fine sugar, 2 oz. Plum flavored brandy) would be a good variation of the classic purple drink. Serve in a Kool Aid pitcher ; one can be purchased on Ebay.
Gourmet potato chips served in a lunch pail would be fun. You could use the lunch pails to serve a variety of items, like Pepperidge Farm cookies or peanut butter cups.
Mini grilled cheese sandwiches (use a variety of cheeses, brie, havarti, swiss, etc.)
A spaghetti salad is another twist on a kid staple that you could serve.

cheesecake lollipops, franks en crute (pigs in a blanket)
chocolate martinis (call them chocolate milk) mini beef wellingtons, mashed potato bar.
